  3. Autoimmune diseases

  4. Blood thinning

  5. Cardiovascular problems

  6. Diabetes

  7. Epilepsy

  8. Eczema

  9. Haemophilia

  10. Hepatitis

  11. Herpes

  12. HIV+

  13. Hyperpigmentation

  14. Infectious diseases/ high fever

  15. Low/high blood pressure

  16. Pacemaker

  17. Pregnant or breastfeeding

  18. Problems with wounds healing

  19. Skin diseases

  20. Taking any medications on daily

  1. Are you currently receiving any treatment from doctor or specialist?

  2. Are you allergic to any medicine, antibiotics, foods or other substances?

  3. Have you ever done any form of Cosmetic tattoo services? Eg Botox, Filler, Laser in the last 4 weeks?
